Adult Primary Care– Christ Community is dedicated to providing for all their patients’ primary healthcare requirements. They know the difficulties many patients encounter when trying to stay healthy. By meeting patients where they are physically and assisting them in maintaining good health, they aim to provide high-quality healthcare.
Board-certified physicians in internal medicine and family medicine make up their staff. To ensure that each patient’s needs are met, their team of nurse practitioners and physician assistants collaborate closely with doctors. In addition to working as a team, they support patients in managing the demands of chronic diseases such as hypertension, diabetes, asthma, COPS, medication-assisted therapeutic interventions (MAT) for opiate abuse, and heart disease.
Behavioral Health–
To ensure they meet all of the patient’s needs, including physical, spiritual, and emotional, integrated behavioral health is a crucial part of the services provided by primary care teams.
Most behavioral health consultants have degrees in psychology, clinical social work, or counseling. They collaborate with primary care doctors and spiritual health advisors to improve care for patients with emotional problems and ongoing medical conditions. When clinically required, patients can meet with a behavioral health consultant as part of their routine primary care visits for a quick assessment and focused intervention.
Down Syndrome Clinic–
The purpose of the Down syndrome clinic is to guarantee that people with Down syndrome receive the care and comprehensive services to advance their development and general well-being. Any person with Down syndrome is welcome to the clinic, which collaborates with the patient’s parents or caregivers to ensure a thorough evaluation.
Family Solutions–
Family Solutions is a section of Christ Community Health Services’ behavioral health unit that uses a family-centered approach to care.
All patients of Christ Community Health Center and school-based health centers may visit the Raleigh Health Center at Christ Community Health Services, which is situated at 3481 Austin Peay Highway. Families can navigate the many challenges they may encounter with their children and within the family in a judgment-free environment at Family Solutions, which is staffed with licensed therapists and psychologists.
General Dentistry–
Christ Community Dental provides various services centered on oral health and patient education to assist patients in maintaining healthy teeth.
HIV Care and Treatment–
In seven of their primary care facilities, they provide HIV medical care, case management, and support services. HIV-positive women can also receive prenatal and obstetric care at their Women’s Health Center. Pre-exposure prophylaxis (PrEP), a treatment that prevents HIV infection, is now being made available at a few select locations.
One of their objectives is to fight HIV-related stigma in the neighborhoods by working with neighborhood churches to provide HIV education and care referrals.
Operation Outreach–
Baptist For people or families without a permanent place to live, Operation Outreach provides free mobile medical care. The Community Alliance for the Homeless clinic has been run by Baptist Memorial Health Care and Christ Community Health Services since 2005.
In Shelby County and the surrounding areas, the mobile clinic—one of Memphis’ biggest providers of health care for the homeless—offers free acute and primary care, disease management assistance, medication, prevention information, and transportation to and from medical appointments. The clinic provides patients with direct access to care and referral services and is staffed by a certified physician and nurse practitioner.
Pediatric Primary Care–
To welcome the baby and support Parents in the care of their newborn, they have pediatricians on call. To assist parents in managing children’s allergies and asthma, they have a board-certified pediatrician with allergy and asthma training. Their pediatricians are available to make pediatric care convenient and comfortable for you and your child, whether for immunizations, well-child exams, sports physicals, or advanced care.
Pharmacy–
Patients can get inexpensive medications from their pharmacies, which are found in Broad Avenue, East Jackson, Frayser, Hickory Hill, Orange Mound, and Third Street health centers, without having to have insurance. The pharmacies serve as a direct conduit between patients and the therapeutic drugs they require to recover. Each year, Christ Community fills more than 179,000 prescriptions. To facilitate care delivery, their pharmacists are also available for in-person consultations with patients and physicians.
School-Based Health Centers–
Christ Community Health Services will cooperate with the student’s insurance companies and the family through their sliding fee program. No student will be denied care because they can’t afford it.
Siloam – Refugee Services–
Christ Community Health Services provides health screenings and immunizations for new self-resettled refugees in Memphis and Shelby County as a Tennessee Clinic Partner for Siloam Health Services, the Statewide Refugee Screening Coordinator for Tennessee. Services are offered within 90 days of a refugee’s entrance into the area.
Social Work–
The Christ Community Health Services’ social work program is a significant addition to the services provided to meet our patients’ primary care needs.
Social workers (at the bachelor’s and master’s levels) and social work interns take care of the needs of their patients in need of social services.
Spiritual Health–
At Christ Community Health Services, they think treating patients holistically means addressing their physical, mental, and spiritual needs. Most health centers have Spiritual Health Advisors who collaborate closely with their medical professionals and staff to provide prayer, support, and spiritual inspiration as part of their healthcare. In addition, they work with nearby churches and communities of faith to build a bridge for patients who want to connect.
