Please follow these instructions to get a copy of your or a family member’s medical records:

Print out the form requesting authorization to release health information.
Fill out the form and sign it legally.
Email [email protected] the completed form.
The form must bear your signature for it to be accepted. Adobe-generated signatures or electronic signatures are not recognized.

Medical records are mailed, not faxed, within 30 days of receiving the signed request.
